  the Equipment Evaluation. Submit the
  Equipment Evaluation Form

• Step 2: We will email you a
  rendered 3D visualization of
  your modified workstation

• Step 3: Give the justification letter and
  quote, provided by Sit-Stand, to your
  employer/decision-maker.
